DESCRIPTION

The Geriatrics Workforce Enhancement Program Coordinating Center’s (GWEP-CC) Age-Friendly Case Study Series presents a range of cases accompanied by podcast episodes developed by Geriatrics Workforce Enhancement Programs (GWEPs) and their partners. The case studies in this series explore three themes in Age-Friendly Health Systems: integrating the 4Ms with electronic health records (EHRs)/electronic medical records (EMRs), tribal, advancing care for tribal organizations and underserved and/or rural (TTOUR) populations, and implementing Age-Friendly practices into nursing home care. Case study authors are from five of the forty-two GWEPs who were funded by Health Resources and Services Administration (HRSA) in 2024.
